Requesting a Change to a Resource in the Resource Databank

 

  1. Search the Resource Databank to confirm the change is necessary.  See Searching the Resource Databank for detailed steps.

  2. Complete the  Medical RDB Request form.

NOTE: Remember to include the Resource ID number for tracking purposes.

  1. Attach the completed Medical RDB Request form and Email it to kdhe.centralofficerdb@ks.gov .

 

Requesting a Change to a Resource on the Active Nursing Facility List

Facilities on the Active Nursing Facility List are handled differently from other RDB changes. If a facility requests a change, email kdhe.ActiveNursingFacilityList@kdheks.gov. Include what the information was, what it is changing to, when it changes, and any other information available. This includes requests to change the name of a facility due to it being purchased by a different company.

 

NOTE:  When a Resource ID is located within the RDB, check the Service type to ensure they are not indicated as a SMART Vendor. Medical Staff do not use resources listed as SMART Vendors. If the only accurate resource found is a SMART Vendor, request an update to a different resource or request that a new Resource is added. . If an appropriate Resource ID is found, note the Resource ID number located in the Resource Databank as it is used to add the admin role to ensure the correct record is used. See Administrative Role.
