Referrals to the PMDT shall be completed through the Contact Log in KEES for each consumer who may qualify for a Presumptive Medical or MediKan determination.
1. Select Contact Log from Utility Navigation.
2. Under Contact Type: Select Document.
3. Under Contacted By: Select Agency.
4. Under Agency: Select KDHE.
5. Under Category: Select Referred to Agency Partner.
6. Under Contact Reason: Select PMDT Contact.
7. Under Additional Notes: Include the following:
a. Applicant Name
b. Application Date
c. Referral Type (Tier 1 and/or Tier 2)
d. Pre-Release Application (if applicable)
e. Inmate Medical Application (if applicable)
8. Click Save.
Once the PMDT has completed a disability determination, notification must be sent to the original eligibility worker via the Contact Log.
1. Select Contact Log from Utility Navigation.
2. Under Contact Type: Select Document.
3. Under Contacted By: Select Agency.
4. Under Agency: Select KDHE.
5. Under Category: Select Referred to Agency Partner
6. Under Contact Reason: Select PMDT Return.
7. Under Additional Notes: Include the following:
a. Outcome of the disability determination.
b. Effective date of the disability determination.
8. Click Save.
