Adding a Test Score Record

  1. Select Services from Global Navigation. Select Appraisal from Local Navigation. Select Test Scores from Task Navigation. The Test Scores List page displays.

  2. Click the Edit button. The Test Scores List page displays.

  3. Select a Type from the drop-down menu.

  4. Select the appropriate Name from the drop-down menu.

  5. Enter a short description of the Test Results (i.e. Passed, Positive, Completed, Able to Work, Referral Needed, etc.).

  6. Enter the test Date using the <mm/dd/yyyy> format or the calendar icon.

  7. Click the Add button to add another record. Otherwise, click the Save button. The Test Score List page displays.
