Select Services from Global Navigation. Select Appraisal from Local Navigation. Select Customer Activities from Task Navigation. The Customer Activities List page displays.
Click the Add Activity button. The Select Activity page displays.
Select an Activity Name from the drop-down menu.
Select a Category from the drop-down menu.
Select a Type from the drop-down menu.
Select a value from the Maximum Distance From Address drop-down menu.
NOTE: The Search parameter is based on the physical address of the consumer.
Click the Search button. The search results display.
NOTE: If the search results are not satisfactory, click the caret to Refine the Search. If the activity does not exist in the Resource Databank, see Requesting an Addition or Change to the Resource Databank.
Select the radio button for the appropriate activity name and click the Select button. The Customer Activity Detail page displays.
Select the consumer’s Name from the drop-down menu.
Select a Program Type from the drop-down menu.
NOTE: If more than one of the same program type is listed, the user must be diligent to select the correct unique identification number for the corresponding program block. See Case Summary Page for more information.
Enter the Start Date using the <mm/dd/yyyy> format or the calendar icon.
Enter the End Date using the <mm/dd/yyyy> format or the calendar icon as appropriate for the activity or for supportive services associated to the activity.
NOTE: A warning displays when the activity dates exceed a 12 month period.
NOTE: End Dates on the Customer Activity Detail page should be in sync with participation recorded on the Activity Progress Detail page. For example, if staff have recorded participation hours on the Activity Progress Detail page for every week in September, the Customer Activity End Date should not be prior to the last day of the month.
Enter the number of hours in the Scheduled Hours per Week for this Activity field.
NOTE: A warning displays when the scheduled hours exceed 79 hours per week.
Verify the Show As Appointment drop-down menu displays as Yes.
Click the Save and Return button. The Customer Activities List page displays.
