This process must start within the context of the Provider Resource Detail. For more information on accessing the Resource Detail page, see Searching the Resource Databank.
Within the context of the relative child care provider Resource Detail, click the Edit button.
In the Basic Information section:
Select Denied from the Status drop-down menu.
Update the Name and/or Payee Name if needed.
NOTE: Enter Relative Child Care Providers First Name Last Name. Enter the middle initial if known without a period. (i.e. John A Smith or John Smith) Hyphens (–) or Apostrophes (‘) may be included if part of the provider’s name.
In the Tax Information section, enter known information:
Select Independent Contractor from the Type drop-down menu.
Enter the ID.
Select the ID Type from the drop-down menu.
In the Addresses section, make needed updates by clicking the Edit button. The Address Detail page displays.
Make needed updates. Click the Submit button. The Select Address page displays.
Click the radio button for the Normalized Address if not already selected.
Click the Select button. The Resource Detail page displays.
In the Phone Information section, make needed updates. The Add button does NOT need to be clicked for the first line of information.
In the Internet Information section, enter an email address if known.
Select Primary from the Type drop-down menu.
Enter the email address in the Address field.
Click the Add button.
Select Not Applicable from the Review Frequency drop-down menu.
Enter 12/31/2999 as the Next Review Date.
Click the Save button. The Resource Detail page displays.
