Certain forms may be mailed directly to collateral contacts. These forms are generated in the same manner as populated information forms. The name and address of the collateral contact are entered instead of the consumer’s information.
Select Document Control from Global Navigation. Select Templates from Local Navigation. The Template Repository Search page displays.
Enter available search criteria. If unsure of specific search criteria, proceed to STEP 3.
NOTE: Keywords are used as search criteria.
Click the Search button. The Template Repository Search page displays with the forms matching the criteria the user entered.
Click the desired form Name hyperlink. The Document Parameters page displays.
Enter the Case Number and click the Go button. This retrieves specific case information for the drop-down fields for the rest of the window.
Select the appropriate Consumer Name, Program, Language and any other mandatory drop-down field based on the form chosen.
Click the Generate Form button. The form displays in PDF format with populated case and office information. Enterable fields are shaded.
Enter the applicable information in the shaded fields.
Enter the Name and Address of the collateral contact.
Review the form for accuracy. Modify or add information in the enterable shaded areas.
Click the Save and Print Locally or Save and Print Centrally button.
Complete a Journal entry.
