Forms are used when a notice is needed for a manual EDBC, EDBC Override or when the system generated NOA is incorrect and cannot be corrected. These forms are referred to as General Notices. It is important to understand that a General Notice is a form that is generated by a user to provide information to a consumer about actions taken.
General notices that are accessible through the KEES Template Repository Search page for DCF use include:
V008 General Correspondence (blank)
If using only the V008, a user should manually generate and sent the KC5720 Rights and Responsibilities form
V115 General Notice of Approval
V200 General Notice of Denial
V400 General Notice of Discontinuance
V600 General Notice of Reinstatement
V700 General Notice of Change
V808 General Correspondence (blank)
NOTE: If no existing form in KEES is available users may use a hard copy of a Non-KEES Notice which is imaged into KEES so it is part of the case file and then mailed.
