Requesting a Change to the RDB - Medical Resource

1. Search the Resource Databank to confirm the change is necessary. See Searching the Resource Databank for detailed steps.

2. Complete the Change Resource section on the Medical RDB Request form.

   NOTE: Remember to include the Resource ID number for tracking purposes.

3. Attach the completed Medical RDB Request form and email it to: KSRDBMedicalRepandFa@maximus.com
