NOTE: Verify the consumer does not qualify for other medical coverage. Verify the consumer has a TB diagnosis and a KDHE developed care plan approved by KDHE Medical Policy Staff.

Complete the following

1. Select Eligibility from Global Navigation. Select Customer Information from Local Navigation. Select Other Prog. Assist. from Task Navigation. The Other Program Assistance List page displays.

2. Click the Add button. The Other Program Assistance Detail page displays.

3. Select the consumer from the Name drop-down menu.

4. Select TB from the Type of Assistance drop-down menu.

5. Enter the Effective Date of Treatment as found on the application using the <mm/dd/yyyy> format or the calendar icon.

6. Enter the Begin Date of the first date of approved program coverage using the <mm/dd/yyyy> format or the calendar icon.

7. Select the appropriate Medical Verified value from the drop-down menu.

8. Click the Save and Return button.
