Changes to the Standard Cut and Paste for 02/01/19

Any form on the Standard Cut and Paste that has the Rights and Responsibilities on it have been updated to reflect the current language.

Spanish fragments and lines about providing interpreters have been removed because of the new Language Assistance form.

There has also been a change in the template used that allows for a fourth page.  NOAS that previously had the instruction to send the KC5725 now have the Rights and Responsibilities language added to the template.  The template is set up to have the same 34-line limit as KEES notices have on their pages.

On forms that had the “If you have questions, please call ____________ at ___________ between ______________.” has been updated with Clearinghouse information

The following fragments have been added and deleted to the Family Medical tab:

Added:

• Removal of premium on CHIP case because of Native American Ethnicity.

Deleted:

• Pregnant Woman Approval
• SSI Recipient Responsibility to notify SSA.

The ES31-70 has been deleted from the Elderly and Disabled Tab as it is available on KDHE-Policy forms
