Check boxes on the ES: 3501 are not consistently remaining selected when the form is sent to the consumer. Until this is fixed, staff can print the form locally and select the checkbox in writing or staff can use the free format check box and space listed under income to type out what is needed as shown below. Reported sections of the form impacted by the issue at this time include the boxes related to Self-Employment Income and Landlord Information.
