Institutional Care to Independent Living

 

NOTE: Change the consumer’s mailing and physical address to the home address. See Contact Summary for information on updating an address.

  1. Select Eligibility from Global Navigation. Select Customer Information from Local Navigation. Select LTC Data from Task Navigation. The LTC Data List page displays.

  2. Click the Edit button for the Institutional Care record that is ending. The LTC Data Detail page displays.

  3. Select Yes from the Discharge Information drop-down menu.

  4. Enter the Discharge Date and any other necessary information from the ES-2126 Notification of Facility Admission/Discharge.

  5. Click the Save and Return button when all information is recorded. The LTC Data List page displays..

  6. Run EDBC for the month Institutional Care ends. See Running EDBC for detailed steps.

  7. Click the Medical hyperlink with a Run Status of Not Accepted on the EDBC List page. The Medical EDBC Summary page displays.

  8. Click the Eligible Budgets hyperlink to check that the LTC Details have accurate start and end dates. If the dates are incorrect the user needs to correct the LTC Data Detail page(s).  The LTC end date is the date prior to the discharge date. Facilities are not paid for the date of discharge.

  9. Click the Close button. The Medical EDBC Summary page displays.

  10. Click the Accept button if the EDBC Result is correct. The EDBC List page displays.

  11. Click the Save and Continue button. The Case Summary page displays.

NOTE: If the consumer had been on a Medical Program with a recipient spouse prior to institutional care or the spouse was receiving income allocation, the worker first needs to end the LTC coverage on the LTC program block, then Rescind or Reapply the consumer on the previous or new program block. See Rescind Program or Reapply Program for more information. Do not reapply coverage on the LTC program block if either the situation applies.

  1.  Add the individual to a usable program block. See Usable Program Block – Medical for detailed instructions.

  2. Add a Requested Medical Type of Medical. See Add/Update Requested Medical Type for detailed instructions.

    1. The Begin Month will be the month following the month of discharge.

  3. Update any applicable Non-Financial Data Collection pages.

  4. Update any applicable Financial Data Collection pages, including the Expense Detail page with the applicable expenses related to the month of discharge.

  5. Run EDBC through the come-up month to process ongoing eligibility. See Running EDBC  for detailed steps.

    1. If the worker can reuse the LTC Program block, use a Timely Notice Exception and a Reason of LTC Late Change through the come up month.

  6. Check system generated notice for accuracy. If necessary, see Manual NOA Generation and Generating a Form for an Institutional Care Facility for detailed steps on sending the notifications.

  7. Select Journal from Utility Navigation.  Record all actions taken on the case.  See Journal for more information about journaling functionality.
