Provider = Select the hospital from the RDB
]Admission Date = Date admitted to hospital
Anticipated Length of Stay = 30 days or more
Current Level of Care = General Hospital
Cost of Care = $9,999
Care Screening Met = Yes
Date Screening Completed = Date Admitted to the Hospital
Discharge Date = Enter discharge date if applicable.
Run EDBC on the LTC program block. Click the Eligible Budget hyperlink and make sure the LTC Details show as IC-GH with the correct start and end dates for each month ran. If the correct information shows, Save and Accept EDBC.
Note: If CHIP has already been approved, a new LTC block would need to be created to overlay the CHIP with the Title 19 (IC/GH).
