This business process shows the steps to process a request for a Trust and/or Annuity Clearance.
The B-6.1: Annuity Clearance Request form is used for Annuities and must be imaged to the case in Perceptive Content.
The B-6.2: Trust Clearance Request form is used for Trusts and must be imaged to the case in Perceptive Content.
Exported trust and/or annuity documents from Perceptive Content (saved as a single PDF. See Exporting Instructions below.)
Trust/Annuity Clearance tasks are not monitored by KDHE-DHCF Policy. Clearance requests must be sent to the following email addresses for completion:
Elderly & Disabled and Long Term Care requests:
E&D.LTC_Trust&AnnuityClearanceRequests@ks.gov
Family Medical Trust & Annuity Requests:
KDHE.MedicaidEligibilityPolicy@ks.gov.
Note: Family Medical clearance requests should only be sent if staff are unable to determine whether the trust/annuity income is taxable. (See PC2022-06-01.)
Access Perceptive Content
Search and Open the document to be exported.
Select File then Export (or click the Export icon). The Export Dialog displays:
Format: Select Single PDF
Directory: Enter File Name (or browse to location and enter the file name) of the document to be exported. NOTE: The location for exports usually defaults to the user’s desktop.
Pages: Indicate the page numbers that are to be exported in the Pages section. (This is important if multiple resource documents are in the same document. Only select the pages specific to the trust or annuity.)
Annotations: Confirm No annotations displays in the Annotations Section.
Click the Options button.
Confirm nothing is selected and click Cancel.
Click OK.
Once the export process is complete, find the document in the location determined in Step 3(b).
Attach the document(s) to the email with the appropriate B.6 clearance request form.
Documentation related to the trust and/or annuity must be exported from Perceptive Content and included as a PDF in the email to KDHE Policy.
Once KDHE Policy has completed the clearance, they will log their findings of countable or not countable on the case.
The worker will determine the value of the trust or annuity and enter necessary records in the KEES in accordance with policy.
Staff need to select the following Application Plan, Capture Profile, and Document Type when imaging the B-6 forms:

Capture Profile: IN Printer - Case No Task
Document Type: Trust/Annuity
Trust information is received for Sarah's Medical Assistance case. The eligibility worker completes the B-6.2: Request for Trust Clearance form and images it to the case in Perceptive Content. The eligibility worker then exports the trust contract documents from Perceptive Content and sends them in an email with the B-6.2 form to KDHE Policy. The KDHE Policy Manager receives the email, evaluates the images and information provided on the B-6.2, and determines Sarah's trust is countable. They log their determination on the case and provide the clearance response to the eligibility worker. The eligibility worker will finish completing the determination on the case including determining the countable value of the trust.
