When Used: When discontinuing Adoption Assistance because the Adoptive Parents are no longer Financially Responsible for the Child

Dear Parent:

This letter is in regard to the monthly Adoption Assistance subsidy you receive on behalf of *CHILD NAME*. Our records indicate that you are no longer financially supporting *CHILD NAME*.  Consequently, your Adoption Assistance case is being terminated, and your payments will cease *MM/DD/YYYY*.

This is in accordance with federal regulations and the Adoption Assistance Agreement you originally signed.

If you feel that this decision is incorrect, please contact your local DCF office.

Estimado padre/madre:

Esta carta se refiere al subsidio mensual de Asistencia para Adopción que recibe a nombre de *CHILD NAME*. Nuestros registros indican que usted ya no mantiene económicamente a *CHILD NAME*.  Por consiguiente, se cierra su caso de Asistencia para Adopción, y sus pagos cesarán el *MM/DD/YYYY*.

Esto se produce en conformidad con normas federales y el Acuerdo de Asistencia para Adopción que firmó originariamente.

Si cree que esta decisión es incorrecta, comuníquese con la oficina del DCF de su localidad.
