Mail Receiver/Imager:
· Triage the mail
o Do not Date Stamp any application without a signature.
o Sort all documents and place Unsigned Applications in their appropriate area.
NOTE: All supporting documentation should be kept with the associated application.
· Attach the P-15: Application Signature Request form and mail the documents back to the consumer. If there is not an address, attempt to contact the consumer for their address. If you cannot contact the consumer, write no address provided on the letter and file the documents in the LIEAP paper retention area.
· Unsigned applications are not date stamped with a received date.
· Unsigned applications are not to be imaged and receipt of the documents will not be logged in KEES. There is no requirement to keep copies or track these documents.
· When a signed application is received without the primary utility account holder’s signature:
o The application is imaged and registered.
o Create a record on the Verification List page using a Verification Detail Type of Application Sig Page. Enter Application Signature is needed in the Description field. Follow the Request for Verification process.
o If the signature page is not returned, negative action any program block registered in error from the unsigned application using a Negative Action Reason of Requested Information Not Received.
o If the signature page is returned, image it to the Correspondence document type.
· When an unsigned application is registered in error:
o Send the P-15: Application Signature Request form and original document back to the consumer.
o Create a record on the Verification List page using a Verification Detail Type of Application Sig Page. Enter Application Signature is needed in the Description field. Follow the Request for Verification process.
o If the signature page is not returned, negative action any program block registered in error from the unsigned application using a Negative Action Reason of Requested Information Not Received.
