Generating a Request Verification Form

 

A verification request form can be generated from the Verification List page.

To generate the form:

  1. Access the case through Person Search or enter the Case Number in the quick search field from Task Navigation and click the Go button.

  2. Click Eligibility from Global Navigation.

  3. Click Customer Information from Local Navigation.

  4. Click Verifications from Task Navigation.  The Verification List page displays.

  5. Click the Generate Form button.  The Document Parameters window displays.

  6. Select the Customer Name from the drop-down menu.

  7. Select the Program from the drop-down menu.

  8. Select the Language from the drop-down menu.

  9. Click the Generate Form button to produce a request form based on pending verifications from the Verification List page.  Click the Generate Blank Template button to produce a blank template.  An Adobe PDF form will display in a separate window with editable fields.
