Individual Demographics - Non-Medical

This page is used to add or update information such as name, social security number, marital status, phone number, etc. for case person(s).

NOTE: The Edit button is always used on the Individual Demographics page as the user is confirming information is correct, as well as possibly entering new information.

To Add or Update Information on the Individual Demographics Page:

  1. Select Eligibility from Global Navigation. Select Customer Information from Local Navigation. Select Individual Demographics from Task Navigation. The Individual Demographics List page displays.

  2. Click the Edit button for the case person the user is updating information for.  The Individual Demographic Detail page displays.  

  3. Confirm the First Name, including spelling.

  4. Confirm the Last Name, including spelling.

NOTE: Name changes are to be documented in two places on the Individual Demographics Detail page. Change the name in the Name section, and then add the previous first and last names in the Previous Name section. This ensures KEES maintains all names associated with the consumer.

  1. Confirm the Social Security Number.

  2. Select the appropriate SSN Status from the drop-down menu. If an SSN status already exists and is considered Verified, do not select a new status.

Use the below guidance when selecting a SSN Status.

NOTE: The SSN Status field defaults to Select when a worker makes a change to the Social Security Number, the worker needs to choose the appropriate SSN Status from the dropdown.

  1. Select the appropriate Marital Status from the drop-down menu.

  2. Select the legal Gender of the consumer from the drop-down menu.

  3. Enter the Date of Birth using the <mm/dd/yyyy> format or the calendar icon.

  4. If the consumer is Active on Medical and Non-Medical programs, do not select a new Verified value from the drop-down menu. Otherwise select the Verified value from the drop-down menu for Date of Birth.

  5. Select the appropriate value from the Is this individual a U.S. Citizen? drop-down menu.

  1. If the consumer is Active on Medical and Non-Medical programs, do not make or change the selection from the Is this individual a Veteran? drop-down menu. Otherwise, select the appropriate value from the drop-down menu.

  2. Select the appropriate value from the Spouse of a Veteran? drop-down menu.

  3. Select the check box(es) for the applicable Race/Ethnicity Origin.

NOTE: If American Indian or Alaskan Native is already checked, do not uncheck.

  1. Select the appropriate value from the Is this person Hispanic, Latino/a or Spanish Origin? drop-down menu.

  2. Complete the Tribe field if applicable.

  3. The Deceased Date is entered for case maintenance by a user or populated by a Batch Interface.  Once a date has been entered and verified, the Verified drop-down menu displays.  After a Deceased Date has been entered and verified, Running EDBC for the appropriate month discontinues assistance for the individual. If the wrong Deceased Date was entered or if a Deceased Date was entered for the wrong person, see Troubleshooting.

  1. Enter the Emancipation Date, if applicable, using the <mm/dd/yyyy> format or the calendar icon.

  2. Select the appropriate Language types from the drop-down menus.

  3. If a Preferred Method of Contact is selected, a Cell Phone Number is required when Text is selected and an Email is required when Email is selected.

  4. Enter Other Communication Needs if applicable.

  5. Select the appropriate value from the Is it OK to call this Individual at Work drop-down menu.

  6. Enter the consumer’s Email, if provided.  If the consumer would like to receive messages via email, select the check box.

NOTE: If a consumer has a SSP account and email address is provided, an email is sent to the consumer to alert them to check the Message Center on the SSP.

  1. Select the Last Grade Completed from drop-down menu.

  2. Enter the consumer’s Phone Number and select the Phone Number Type from the drop-down menu.  

  1. Use the Previous Names block to capture Alias and Maiden Name information.

  1. The Previous Social Security Numbers block is only used to record past Social Security Numbers that were issued by Social Security as per KEESM 2130 (for example, adoption cases).

NOTE:  If a consumer is using a Social Security Number that does not belong to them, add a Case Flag and Journal the information.

  1. Indicate if the consumer is a dangerous person by selecting Yes from the Dangerous Person Indicator drop-down menu.

NOTE: An asterisk displays next to the person’s name on the All People Associated with the Case block on the Case Summary page when Yes is selected.

NOTE: The MCO Choice drop-down menu is not used by Non-Medical programs.

  1. Click the Save and Return button.
