Individual Demographics - PPS

  1. Select Eligibility from Local Navigation and Individual Demographics from Task Navigation.  The Individual Demographics List page displays.

  2. Click the Name hyperlink to view the Individual Demographics Detail page.

  3. Verify the following auto-populated fields are accurate:

NOTE:  The SSN Status may already be populated.  If SSN Provided is displayed, continue with STEP 4.  If verifications are obtained from the HUB or SSA-SVES, the SSN Status will auto-populate and is not to be changed by the worker.  Individual Demographics is a shared page between programs.  This information may already be populated.  Refer to EES Non-Financial Data Collection Individual Demographics for further information.

  1. Select the appropriate value from the Name Type drop-down menu.

NOTE:  The Name Type has been added to communicate with Child Support Services when the child's status changes from birth name to adoptive name.

  1. Select Good Cause from the SSN Status drop-down menu.

NOTE:  If the child had prior medical, the SSN Status may display HUB-SSA or SSA-SVES.  If one of these values displays the user is to leave those values in the display field.

  1. Select the appropriate Gender from the drop-down menu.

  2. Select Verified from the Verified drop-down menu associated to the Date of Birth field.

  3. Select the appropriate value from the Is this individual a U.S. Citizen? field. If Yes is selected a Verified drop-down menu dynamically displays.

  4. Select Verified from the Verified drop-down menu associated with the Is this individual a U.S. Citizen? field.

  5. Select the check box for the appropriate Race/Ethnic value.

  6. Select the appropriate value from the Is this person Hispanic, Latino/a or Spanish Origin drop-down menu.

  7. Update the Spoken Language, if appropriate. The system default is English.

  8. Select the appropriate value from the Written Language drop-down menu.

  9. Select the appropriate value from the Language of Correspondence drop-down menu.

  10. Enter telephone number of the adoptive parent, guardian or youth in the Phone Number text box.

NOTE: Telephone numbers should only be entered for Adoption Subsidy, Permanent Custodian Subsidy, Youth Independent Living, and PPS Vendor Payments.  

  1. Enter information about Previous Names including:

NOTE:  The Name Type has been added to communicate with Child Support Services when the child's status changes from birth name to adoptive name.

  1. Enter Previous Social Security Numbers, if applicable.

  2. Select the appropriate value from the MCO Choice drop-down menu for medical programs. If the user wants to keep a sibling group not already known to KEES with the same MCO.

NOTE:  If an MCO has already been selected on an existing case, it will carry forward.  If it is a new case or the MCO is blank for any reason, the system randomly selects one.  The user does not need to select an MCO.

  1. Click the Save and Return button.  The Individual Demographics List page displays.
