For those youth in the custody of DCF or KDOC-JS who move into their own apartment.
Access the case through Person Search or enter the Case Number in the quick search field from Task Navigation and click Go. The Case Summary page displays.
Select Eligibility from Global Navigation. Select Customer Information from Local Navigation. The Case Summary page displays.
Select Case Summary from Local Navigation. Click the View Details button on the Case Summary page.
Click the Edit button. The Medical Program Detail page displays.
Click Edit for the Placement Provider name.
Enter the date the prior foster home placement ended for the End Date field using the <mm/dd/yyyy> format or calendar icon.
Click the Save and Return button. The Medical Detail page displays with the child only in the Administrative Roles section.
Click Add in the Administrative Roles section.
Select Placement Provider from the Administrative Role drop-down menu.
Click the Select button under Resource. The Select Resource page displays.
NOTE: The Contract Manager Agency or KDOC-JS office will be added as the placement provider. The agency may need to be added to the Resource Databank.
Click the Search button. The Select Resource page displays with results.
Click the radio button by the correct resource.
Click the Select button. The Administrative Role Detail page displays.
Enter the date of placement for the Begin Date using the <mm/dd/yyyy> format or calendar icon. The Begin Date must be the date of the new placement in an apartment.
Click the Save and Return button. The Medical Program Detail page displays
Click the Save and Return button. The Case Summary page displays.
Select Contact from Task Navigation. The Contact Summary page displays.
Add the new address of the youth. See Contact Summary - PPS for detailed steps.
Click the + next to Foster Care from Task Navigation. Task Navigation expands to display the Foster Care data collection pages.
Select Child Placement from Task Navigation. The Child Placement List page displays.
Click the Add button. The Child Placement Detail page displays.
Click the Select button associated to the Placement Name. The Select Foster Care Resource page displays.
Enter as much information about the Independent Living program as possible. Click the Search button.
The Select Foster Care Resource page redisplays with a list of resources.
Select the radio button for the appropriate resource. Click the Select button. The Child Placement Detail page displays with the Placement Name populated.
Select Case Management Agency from the Payee drop-down menu. The Select button dynamically displays.
Click the Select button associated to the Payee Name. The Select Foster Care Resource page displays.
Enter as much information about the Case Management Agency as possible. Click the Search button. The Select Foster Care Resource page redisplays with a list of resources.
Select the radio button for the appropriate resource. Click the Select button. The Child Placement Detail page displays with the Payee Name populated.
Enter the date the child entered Independent Living in the Begin Date field using the <mm/dd/yyyy> format or the calendar icon.
Click the Save and Continue button. The Effective Dating Confirmation List page displays with a Begin Date for the new placement and the Begin Date and End Date of the previous placement.
Click the Save button. The Child Placement List page displays with a record showing the Independent Living placement.
Select Run EDBC from Task Navigation.
Select the month the child entered Independent Living in the Benefit Month field and click the Select button.
Select the check box for Foster Care and click the Run EDBC button. The EDBC List page displays.
Click the Foster Care hyperlink with a Run Status of Not Accepted. The Foster Care EDBC page displays.
Verify the information is correct.
If the information is correct, click the Accept button. The EDBC List page displays with a Run Status of Accepted-Not Saved. Proceed to STEP 38.
If the information is not correct, click the Cancel button. Return to the data collection pages to make corrections.
Click the Save and Continue button. The Case Summary page displays.
Select Journal from Utility Navigation. Record all actions taken on the case. See Journal for more information about journaling functionality.
