Change in Foster Care Placement

A child in Foster Care may change placements often including moving to another foster home, a group home or other types of placements. Placement changes may require updates to the PPS Medical, Foster Care and Adoption Assistance cases.

To Change Foster Care Placement

  1. Access the case through Person Search or enter the Case Number in the quick search field from Task Navigation and click Go.  The Case Summary page displays.

  2. Select Contact from Task Navigation.  The Contact Summary page displays.

NOTE:  An address change for a child in foster care will generally only occur when the Custody Authority changes, e.g. from DCF to KDOC.

  1. Click the Add button if an address change is needed.  The Address Detail page displays.  See Contact Summary - PPS for more information.

  2. Click the + for Foster Care from Task Navigation to expand the Foster Care pages. Select Child Placement from Task Navigation. The Child Placement List page displays.

  3. Click the Add button. The Child Placement Detail page displays.

  4. Click the Search button associated with the Placement Resource. The Resource Search page displays.

  5. Enter as much information about the new placement as possible. Click the Search button. The Resource Search page redisplays with a list of resources.

  6. Click the radio button next to the appropriate resource and click the Accept button. The Child Placement Detail page displays with the resource populated.

  7. Select Case Management Agency from the Payee drop-down menu. The Search button dynamically displays.

  8. Click the Search button. The Resource Search page displays.

  9. Enter as much information as possible for the Case Management Agency.

  10. Click the Search button. The Resource Search page displays with a list of resources.

  11. Click the radio button for the appropriate resource and click the Accept button. The Child Placement Detail page displays with the Case Management Agency populated in the Payee field.

  12. Enter the date of the new placement in the Begin Date field using the <mm/dd/yyyy> format or calendar icon.

  13. Click the Save and Continue button. The Effective Dating Confirmation List page displays with the Begin Date of the new placement and the Begin Date and End Date of the previous placement.  

  14. Click the Save button. The Child Placement List page displays with a record showing the new placement high-dated.

  15. Select Run EDBC from Task Navigation. The EDBC List page displays.

  16. Select the month of the placement change from the Benefit Month drop-down menu. Click the Select button.

  17. Select the check box for Foster Care and click the Run EDBC button. The EDBC List page displays with a Foster Care hyperlink showing a Run Status of Not Accepted.

  18. Click the Foster Care hyperlink. The Foster Care EDBC page displays.

  19. Verify the information on the Foster Care EDBC page is correct.

  1. Click the Save and Continue button. The Case Summary page displays.

  2. Click the View Details button in the Medical Programs block.  The Medical Program Detail page displays.

  3. Click the Edit button.  The Medical Program Detail page displays with edit capabilities.

  4. Click the Edit button on the line for the existing Placement Provider in the Administrative Role section.  The Administrative Role Detail page displays.

  5. Enter the date the prior foster care placement ends in the End Date using the <mm/dd/yyyy> format or the calendar icon.

NOTE:  There should not be two administrative roles for Placement Provider open at the same time.  The End Date for the existing foster home is generally the date before the consumer is placed in the new foster home.

  1. Click the Save and Return button.  The Medical Program Detail page displays.

  2. Click the Add button.  The Administrative Role Detail page displays.

  3. Select Placement Provider from the Administrative Role drop-down menu.  The Resource label displays.

  4. Click the Select button under Resource.  The Select Resource page displays.

  5. Select Foster Care from the Category drop-down menu.

  6. Enter as much information regarding the new foster care placement as possible.

  7. Click the Search button.  The Select Resource page displays with results.

  8. Click the radio button by the correct resource.

  9. Click the Select button.  The Administrative Role Detail page displays.

  10. Enter the date of placement for the Begin Date using <mm/dd/yyyy> format or the calendar icon.  

NOTE:  The Begin Date must be the date of the new placement.

  1. Click the Save and Return button.  The Medical Program Detail page displays.

  2. Click the Save and Return button.  The Case Summary page displays.

NOTE: EDBC does not have to be completed for Medical. Contact information will be updated with MMIS through a nightly batch.

  1. Select Journal from Utility Navigation.  Record all actions taken on the case.  See Journal for more information about journaling functionality.

  2. Image the Acknowledgement to the case in Perceptive Content. See Imaging.
