A verification request form can be generated from the Verification List page.
Access the case through Person Search or enter the Case Number in the quick search field from Task Navigation and click Go. The Case Summary page displays.
Select Eligibility from Global Navigation. Select Customer Information from Local Navigation. Select Verifications from Task Navigation. The Verification List page displays.
Select the appropriate search parameters and click the Search button.
Select which verifications should populate on the verification request form using the check boxes.
Click the Generate Form button. The Document Parameters window displays.
Enter the Case Number and click the Go button.
Select the Customer Name from the drop-down menu.
Select the Program from the drop-down menu.
NOTE: One form may be sent for multiple programs. The user may select TANF as the program, but still generate the form for Food Assistance and/or Child Care also.
Select the Language from the drop-down menu.
Click the Generate Form button to produce a request form based on pending verifications from the Verification List page. Click the Generate Blank Template button to produce a blank template. An Adobe PDF form displays in a separate window with editable fields.
